Q. If I locate a registered dietitian (RD) on this web site, does that mean that my visit to that RD will be paid by my insurance plan?

A. No. This site lists what plans we are a provider of nutrition services for. We do not have information about benefits or payment levels.

Q. How do I get help to find out my health care coverage and explanation of benefits?

A. To get the information you need about a your coverage, please contact the customer service department at your health plan through the phone number located on your member ID card, or in your enrollment materials.  Insurance representative here are key questions to ask:

    • Do they accept the codes we use for medical nutrition therapy is 97802 and 97803 and 97804.
    • If no what codes are acceptable for nutrition counseling since the codes used by the government states the above codes. List:__________________
    • Does your plan cover for the specific reason or diagnosis you plan to be seen on our office? Example if you are overweight, will it be covered? Only Medical Conditions?
    • If they cover the above, how many visits per year do they allow?
